We have limited space! A participant is registered
only when the following is completed and received by us on a
first-come, first-served basis, according to the time/date of
receipt by on-line/e-mail, 617-812-0363 (fax), or from the Post Office.
Please note that we cannot finish processing your registration or reserve a spot for your child until your
registration is 100% complete.

Measles, Mumps, & Rubella (MMR): At least 1 dose of MMR vaccine, and/or
1 dose of live measles vaccine or a second MMR, are required for all campers entering grades K-12 or
college in the fall of 2010. |

Polio: At least 3 doses of either trivalent oral polio
vaccine (OPV) or an enhanced potency inactivated polio vaccine (e-IPV). If a mixed schedule of polio (IVP
& OPV), then 4 doses are required. |

Diptheria and
Tetanus Toxoids and Pertussis*: At least 4 doses of DtaP/DTP/DT/Td
vaccination are required or, 3 doses of Td (Persons
7 yrs or older needing additional vaccines to comply
with above. Td is to be used)
* Booster dose of Td:
- grades 7-10 need booster if >5yrs since last dose
of DTaP/DTP/DT
- grades 11-12 need booster if more than 10 yrs
since last dose of DTaP/DTP/DT/Td (Tdap is also acceptable) |

Hepatitis B: Anyone born after January 1, 1992, 3 doses of
hepatitis B vaccination are required. |
| Please note: The Tennis Academy at Harvard operates under the
strict guidelines of the Massachusetts Department of Public Health and the City of Boston Department of
Public Health, which allow no exceptions under any circumstances. These required medical forms and immunizations are almost
always taken care of when children enroll in school each year. However, no child will be allowed
to participate in our academy without first meeting all requirements or presenting a signed, current religious
exemption form - no exceptions. |
- Medical Insurance Information No registration will be considered
complete without this documentation.
- Non-Refundable Full Payment is the final step in being 100% complete.

Discounts, Deadlines, and Re-enrollment:
Discounts:
Early Enrollment Discount: If your registration
is received and the five steps are completed by January 31, 2010
(website date), you will receive a $25 per week tuition discount per child.
Discount for Additional Weeks: After purchasing
the first week of camp, you will receive a 10% discount on subsequent
weeks.
Note: These discounts do not apply to lunch fees, merchandise
or cancellation insurance.
These discounts do not exclude one another (i.e., if you register for 2 weeks or more, and complete your
registration by January 31, 2010, you will receive both discounts).
Please note: To qualify for these discounts, all 5 steps of
your registration must be completed and received by our office.
Deadlines:
Initial Registration: all registration materials - all medical
information, and payment - must be in our office by 5:00 P.M.
the Wednesday before the desired week of enrollment, subject to availability.
Re-enrollments: To re-enroll for more weeks, we must be notified
by Wednesday at 5:00 P.M. the week before the desired week in person via
phone 617-678-0142, or e-mail: info@thetennisacademy.com. You may also add weeks of tennis fun on-line!
Just log into your account and follow the easy steps.
Emergency Registration: To ensure that we have sufficient time
to complete our curriculum planning and meet our staffing needs,
we have a firm deadline of 5:00 P.M. on the Wednesday before the week of enrollment
or re-enrollment. However, since emergencies do arise, we may
be able to accept a camper after this deadline - if there is an
appropriate space available - for an additional fee of $125.
Cancellation Insurance / Cancellations:
Cancellation Insurance:
Since your plans may change, we offer Cancellation Insurance,
which may be purchased for the total number of weeks of enrollment
only (there is no per week Cancellation Insurance offered), at
a cost of $30/week (i.e. $30 times the total number of weeks
enrolled) at the time of the initial signup only. With Cancellation
Insurance, if you cancel 14 days or more before the start of
a session, you will receive a refund to your method of payment (minus
the cost of Cancellation Insurance). If you cancel within
14 days of the start of a session you will receive a junior
camp credit, good for any immediate family member valid through
the summer 2011 (i.e., an additional year of camp credit) rather than a refund. This
credit may not be used for the adult camp.
Cancellations:
If you have to cancel and have not purchased Cancellation Insurance,
you will receive a junior camp credit - rather than a refund
- valid through the summer of 2010. Junior camp credits cannot
be applied to adult programs or be used by another family member
or person.
Participants from Outside the United States
Campers from outside the US are required to have the same medical, immunological, and insurance information as
domestic campers. This information must be written in English or be accompanied by an English translation.
Your medical insurance provider must also supply a letter, in English, stating that the participant is covered by
his/her medical insurance when in the United States, and during the time of his/her participation at the camp.
Please remember that obtaining, translating, and forwarding appropriate medical forms from outside the U.S. can
take a great deal of time. Since no camper can participate in the program in any way without all medical documentation
being on file, please plan ahead to avoid disappointment!
